Architect of a new standard in implant dentistry
Dr Alex Fibishenko built a reputation treating complex dental implant cases. Now, the founder of All-On-4 Clinic is taking his patient-centred full-mouth rehabilitation model beyond Australia, with international expansion in sight.

For decades, patients requiring full-arch dental implants often faced an uncertain journey: inconsistent treatment standards, lengthy procedures and varying outcomes depending on who performed the work. But Australian dental implant surgeon, Dr Alex Fibishenko, believed there was a better way.
Motivated to pursue implant dentistry after watching his grandmother struggle with dentures, Dr Fibishenko went on to spend his career making full-arch implant rehabilitation more predictable, more accessible and less daunting for patients. He is the founder of the All On 4 Clinic brand, and the rapidly expanding All-on-4-Plus dental franchise network across Australia, which prioritises optimal long-term outcomes, immediate function, and simplified patient journeys.
However, Dr Fibishenko believes it’s not just about the journey or the destination, but mostly about the company, the surgeon. “There’s no recognised specialty for implants, so patients often don’t know where to go,” he says.
Practising exclusively in oral implantology and dento-facial surgery, Dr Fibishenko has performed thousands of implant procedures with tremendous success to become one of Australia’s best-known exports in the field, and a globally-renowned key opinion leader and educator, influencing dentists, specialists and university professors and implant manufacturers around the world with his pioneering work.
As one of the pioneers of the All-on-4 concept, back in 2002, Dr Fibishenko began developing protocols to improve the outcomes and patient experiences with full arch dental implants by moving away from the traditional 2-stage approach to avoid the biologic and long-term impacts on the implants when fitted with a less-than-ideal temporary set during the critical initial healing. He invented the All On 4 Plus system, using what he calls “immediate final teeth” – refined permanent restorations fitted within 24 hours rather than months later. By 2006, that protocol had become standard practice within his clinic and a point of interest for colleagues from around the globe.
“We identified very early that temporary teeth were contributing to many of the problems that we were seeing with implants at the time,” he says. “Other than the inconvenience and struggles of patients between the stages, temporary teeth are fragile, so that any cracks can result in the disconnection of individual implants from the bridge, resulting in overload. They are also not as refined as final teeth, and the rough surfaces accumulate plaque and debris, resulting in inflammation, gum shrinkage and other complications, most of which are irreversible. Instead, our patients have surgery one day, and their final teeth the next. When the implants are fitted with more refined immediate restorations during healing, the biomechanical burden is substantially reduced. It’s just common sense.”
This shift in thinking and practice required tighter integration between surgery, digital planning, prosthodontics, and laboratory manufacturing. But the real innovation, he says, was building a scalable treatment ecosystem capable of delivering consistent outcomes across multiple clinics and clinicians.

“I wanted to create an environment where the people involved are true experts in this area,” Dr Fibishenko says. “That requires the right facilities, the right people, the right systems and a huge amount of commitment.”
Dr Fibishenko opted for a franchise-style partnership approach, mentoring and working with experienced practitioners who already had suitable infrastructure and surgical expertise.
“The All On 4 Plus system itself is important in terms of why we’re able to scale,” he says. “You need to have something genuinely different to offer.”
The franchise model began in 2014 with five clinicians across Perth, Sydney and Melbourne. Growth was intentionally restrained in the early years while Dr Fibishenko focused on refining workflows and training senior clinicians who could eventually become trainers themselves.
In 2020, during the COVID-19 outbreak, Dr Fibishenko was able to dedicate more time to working with new franchise partners and clinicians, who were expanding rapidly across the country. Now, 27 All-On-4 Plus Premium Providers across Australia operate using the All-On-4 Plus system and protocols.
In some cases, existing dental clinics are retrofitted with dedicated implant suites resembling hospital-style surgical environments. In others, new facilities are developed around the All-On-4 Plus model.
Most importantly, every franchise clinic operates through integrated patient-management software, detailed risk profiling and standardised workflows designed to improve consistency across practitioners and locations.

“I wanted to remove variability as much as possible, not in the colour or shape of teeth people might want – that’s completely individualised, but in the safety and standards of care, we have strict protocols for that,” Dr Fibishenko says.
Being truly patient-centric is part of All-on-4-Plus’ ethos. Often, patients arrive after years of frustration and uncertainty – or being told their case is too complex.
“A significant part of our work has been addressing complex, non-ideal cases,” he says. “Across our network, my fellow senior associates and I have treated many patients who were previously told they were unsuitable candidates for implants. Through our dedication specifically to this area of surgical dentistry and extensive experience with these challenging cases, we’ve been able to provide solutions that others could not. In most instances, these patients leave our clinics with permanent fixed teeth within 24 hours of surgery. For the patient, the process is simpler, more cost-effective, and delivers a more predictable outcome than many traditional treatment approaches.”
The treatment itself can be transformative, particularly for patients affected by severe gum disease, extensive decay, trauma, or failing dental work. Beyond restoring oral function, it often has a profound impact on confidence, wellbeing, and overall quality of life.

Dr Fibishenko says he personally trains franchise clinicians across surgical procedures, restorative techniques and operational systems, and his mentorship remains central to maintaining consistency. That’s also an attractive offer for franchisees.
“We don’t offer this intensive type of mentorship outside the franchise. That’s one of the reasons people want to join,” he says.
Looking ahead, Dr Fibishenko is set to scale internationally. “I’ve had multiple dentists and specialists from abroad wanting to collaborate,” he says. “This is another level of commitment on my part, but I think I’m ready.”
